Is gender discovered when one looks inside one's undergarments, or is gender undetermined until one discovers it?

The Setup

shinsei has always been an uncomfortable girl. Shy, always seeming to overcompensate for something, aggressive to a fault, violent, frequently depressed, irritable, and a whole slew of other problems.

Well, shinsei is much more comfortable as a boy. So, shinsei decided that he was going to be comfortable.

He did this without changing anything except his own perceptions of himself. He's rather adamant about masculine pronouns in reference to himself, and refuses to change his body any. He's very happy with his feminine body.

It is still undetermined how others will react to this.
